Anthem Blue Cross Blue Shield – Health Insurance
Constellation Schools offers two health insurance plans for employees to choose from. Both plans are PPO’s with prescription drugs covered after co-pays. For current “Blue Access” Network Providers, you may refer to www.anthem.com.
Humana Dental Insurance Co. - Dental Insurance
This PPO Dental Plan includes preventative, basic, and major dental services. Orthodontia for children is also covered. Humana also provides discounts for eye exams and vision hardware. For current network provider listings, please refer to http://www.humanadental.com/.
Humana Life Insurance Co. – Life Insurance
A life insurance policy in the amount of $25,000 is issued to each eligible employee. There is “no cost” for employees to enroll. Beneficiaries are named by each employee.
Humana Life Insurance Co. – Voluntary Life InsuranceAdditional life insurance is available to employees of Constellation Schools and their families. Guaranteed issue limits are up to $85,000. Premiums are determined at the time of purchase. “No medical questions!!”
Payroll Deductions
All medical and/or dental premium contributions are made by “pre-tax” payroll deduction!
Humana Life Insurance Co. – Life Insurance
A life insurance policy in the amount of $25,000 is issued to each eligible employee. There is “no cost” for employees to enroll. Beneficiaries are named by each employee.
AFLAC
AFLAC policies pay direct cash benefits in case of accident or ilness. Optional plans include Disability Personal Accident, Specified Health Event Protection (Heart Attack, Stroke, Kidney Failure, Coronary Artery Bypass, etc.), Personal Sickness, Cancer Indemnity, Hospital Protection, etc. All AFLAC premium constributions are made by "after-tax" payroll deduction.
Retirement
Constellation Schools participates in both SERS (School Employees Retirement System of Ohio) and STRS (State Teachers Retirement System of Ohio). Click to view more information regarding these retirement programs.
Benefit Time
Sick Time: All full time Constellation Schools employees are given 1.25 sick days per month, up to 120 days.
Personal Time: All full time Constellation Schools employees are given 2 Personal Days at the beginning of each fiscal year.
Vacation Time: All full time, year round, Constellation Schools employees are given Vacation Time. The number of weeks is determined by job category.
